SHORE UP! Inc. is seeking a contractual Licensed Social Worker for our Adult Day Services and Residential Services programs.   This Licensed Social Worker will:

  • Provide ongoing recruitment to combat attrition in the programs.  Recruit new participants by networking with community services, health services and social services providers.
  • Provide orientation and in-service training for staff, students and volunteers.
  • Provide available resources and eligibility criteria to participants and their families while assisting them in gaining appropriate care.
  • Make initial home visits with the Registered Nurse to assess individuals while collecting necessary admissions information.
  • Maintain confidential participant records of services and needs.
  • Coordinate and implement individual, group and family counseling.
  • Ensure awareness of state and local regulations and policies regarding the programs, as well as potential impact on participants.
  • Provide discharge and referral services to participants.

SHORE UP! Inc. is seeking a contractual RN to provide skilled nursing care for our Adult Day Services and Residential Services programs. This RN will:

  • Provide skilled nursing care for participants based on home visit, assessments and problems identification.  This includes consultant, emergency services and documentation within clinical records.
  • Develop a written plan of care for each participant in consultation with their physician.  Provide evaluations and updates in accordance to current regulations.
  • Leads, evaluates, guides, informs and supervises assigned staff in collaboration with the center director.
  • Coordinates with necessary agencies and/or physicians to certify participants through the Professional Service Review Organization, Adult Evaluation and Review Services and Veterans Administration.
  • Reviews Activity Program and calendar accordingly.  Involves and educates staff in care planning implementation and ongoing participant review and progress.
  • Administers medication and treatment during center operation hours, as ordered by the participant physicians.
  • Develop and maintain cooperative relationships with community agencies and organizations.  Educates community agencies and groups regarding the SHORE UP! Adult Day Services program and goals.
